Resumen Introducción: Las familias de personas menores con síndrome de zika congénito (SZC) viven con dificultades para satisfacer sus necesidades de salud, por lo que hacen un llamado al Poder Judicial para gozar de este derecho. Objetivo: Conocer las principales motivaciones de las demandas presentadas por madres de niños y niñas con SZC y sus desenlaces. Metodología: Estudio documental exploratorio con enfoque cualitativo, realizado en el sitio web de JusBrasil y recogido en marzo de 2020. Se incluyeron 15 juicios publicados entre enero de 2016 y junio de 2019 en JusBrasil. Se realizó el análisis léxico a través del software IRaMuTeQ y el análisis de contenido temático. Resultados: El acceso a las tecnologías asistenciales es la principal motivación de las demandas, con el fin de asegurar la mejora en el desarrollo de la persona menor y su consecuente independencia. Las decisiones judiciales beneficiaron a niñas y niños con SZC, basadas en los derechos a la salud, a la vida y protección y a ir y venir. Conclusiones: Para mitigar los impactos de la judicialización en salud, las autoridades sanitarias y judiciales pueden invertir en una mejor vigilancia y seguimiento de los factores de riesgo y morbilidades, rigor en los protocolos sanitarios que implican la migración de personas en zonas fronterizas, ofrecer condiciones ambientales y habitacionales dignas, realizar cuidados preventivos, con énfasis en la eficiencia de la inmunización, además de la organización y operación de una red de atención de salud efectiva con enfoque interdisciplinario.

Resumo Introdução: As famílias de crianças com Síndrome Congênita do Zika (SCZ) convivem com dificuldades para suprir suas necessidades de saúde, portanto acionam o poder judiciário para gozar do seu direito à saúde. Objetivo: Apreender as principais motivações das impetrações judiciais requeridas por mães de crianças com SCZ e seus desfechos. Metodologia: Estudo exploratório documental com abordagem qualitativa, realizado no sítio eletrônico JusBrasil e coleta procedida em março de 2020. Foram incluídas 15 impetrações judiciais publicadas entre janeiro de 2016 e junho de 2019. A análise lexical através do software IRaMuTeQ e a análise de conteúdo temática foram realizadas. Resultados: O acesso às tecnologias assistivas é a principal motivação para impetrações judiciais, com vistas a assegurar melhora no desenvolvimento da criança e consequente independência da criança. As decisões judiciais beneficiaram as crianças com SCZ, fundamentadas no direito à saúde, direito à vida e proteção, e o direito de ir e vir. Conclusão: Para mitigar os impactos da judicialização da saúde as autoridades sanitárias e judiciárias podem investir em melhor vigilância e monitoramento dos fatores de risco e morbidades; rigor nos protocolos sanitários que envolvem migração de pessoas em zonas fronteiriças; ofertas de condições ambientais e de moradia dignas; realização de cuidados preventivos com destaque para a eficiência da imunização; além da organização e funcionamento de uma rede de atenção à saúde eficaz com abordagem interdisciplinar.

Abstract Introduction: Families of children with Congenital Zika Syndrome (CSZ) live with difficulties to meet their health needs; therefore, they resort to the law system in order claim their right to health. Objective: To apprehend the main motivations and outcomes of the lawsuits filed by mothers of children with CSZ. Methodology: Exploratory documentary study with a qualitative approach carried out on the JusBrasil website and collected in March 2020. The study included 15 lawsuits published between January 2016 and June 2019 in JusBrasil. A lexical analysis through the IRaMuTeQ software and a thematic content analysis were performed. Results: The access to assistive technologies is the main motivation for the lawsuits; these are issued with the objective to ensure improvement in the child's development and further independence of the child. Court decisions benefited children with CSZ based on the right to health, the right to life and protection, and the right to come and go. Conclusion: To mitigate the impacts of health judicialization, health and judicial authorities can invest in better surveillance and monitoring of the risk factors and morbidities, strictness in the health protocols that involve migration of people in border areas, offering of decent environmental and housing conditions, execution of preventive care with emphasis on the efficiency of immunization, as well as the organization and execution of an effective health care network with an interdisciplinary approach.

ABSTRACT Objective: To compare the glucose metabolism of patients with chronic hepatitis C virus infection treated with direct-acting antivirals (DAAs) in pretreatment and sustained viral response (SVR) periods. Materials and methods: This was an intervention pre-post study of 273 patients with chronic hepatitis C virus infection treated with DAAs from March 2018 to December 2019. Glycidic metabolism was evaluated through homeostasis model assessment (HOMA) – insulin resistance (IR) and HOMA-β indices and assessments of insulinemia and HbA1c levels. These parameters were analyzed with a T test by paired comparison of the means of the variables and Wilcoxon's test paired for the median; in the variables with an abnormal distribution, the Z score was generated for the mean in both the pretreatment and SVR periods. Statistical significance was considered at p ≤ 0.05. Results: Among 273 participants, 125 (45.8%) had prediabetes, and 50 (18.3%) had diabetes. In SVR, there was a significant increase in platelets, albumin, alkaline phosphatase, cholesterol and triglycerides and a significant decrease in aspartate aminotransferase, alanine aminotransferase, gamma GT and bilirubin. The HOMA-IR and HOMA-β indices increased in SVR from 1.95 to 2.29 (p = 0.087) and 71.20 to 82.60 (p = 0.001), respectively. Insulinemia increased from 7.60 μU/mL to 8.90 μU/mL (p = 0.011). HbA1c decreased from 5.6 to 5.4 (p < 0.001). Among patients with prediabetes and those with diabetes, the reduction in HbA1c values was significant (p = 0.006 and p = 0.026, respectively). Conclusion: SVR significantly impacts and leads to improvement in glucose metabolism in patients with chronic liver disease induced by hepatitis C virus.

Abstract Objective: This article aims to review the literature regarding the use of technologies to promote mental health for pregnant women. We seek to: understand the strategies that pregnant women use for mental health care. Also, we investigate the existence of scientific evidence that validates such practices. Methods: This study follows the PRISMA guidelines for systematic reviews. We analyze 27 studies published between 2012 and 2019. We include publications in Portuguese, English, and Spanish. Results: The results revealed several different possibilities to use technology, including the use of text messages and mobile applications on smartphones. Mobile applications are the most commonly used approaches (22.5%). Regarding the strategies used, cognitive-behavioral approaches, including mood checks, relaxation exercises, and psychoeducation comprised 44.12% of the content. Conclusion: There is a need for further investigation and research and development efforts in this field to better understand the possibilities of intervention in mental health in the digital age.

Resumo Objetivo: Este artigo objetiva revisar a literatura quanto ao uso das tecnologias como promotoras de saúde mental de gestantes. Desta forma, compreender quais são as estratégias utilizadas no cuidado da saúde mental dessas mulheres, assim como verificar se há evidências científicas que justifiquem a implementação dessas práticas. Métodos: Este estudo segue o protocolo PRISMA para revisões sistemáticas de 27 estudos publicados em 2012-2019, incluindo publicações em português, inglês e espanhol. Resultados: Os resultados revelaram diferentes possibilidades de utilização da tecnologia, sendo o uso de mensagens de texto e de aplicativos em smartphones mais os utilizados (22,5%). No que se refere às ferramentas utilizadas, estratégias cognitivo-comportamentais, tais como verificação do humor, exercícios de relaxamento e psicoeducação compreenderam 44,12% do conteúdo. Conclusão: Verifica-se a necessidade de mais investimentos nessa área para que se possa compreender as possibilidades de intervenção em saúde mental na era digital.

Abstract Objective: To assess the predictive value of selected growth phenotypes for neonatal morbidity and mortality in preterm infants < 30 weeks and to compare them with INTERGROWTH-21st (IG21). Methods: Retrospective analysis of data from the Brazilian Neonatal Research Network (BNRN) database for very low birth weight (VLBW) at 20 public tertiary-care university hospitals. Outcome: the composite neonatal morbidity and mortality (CNMM) consisted of in-hospital death, oxygen use at 36 weeks, intraventricular hemorrhage grade 3 or 4, and Bell stage 2 or 3 necrotizing enterocolitis. Selected growth phenotypes: small-for-gestational-age (SGA) defined as being < 3rd (SGA3) or 10th (SGA10) percentiles of BW, and large-for-gestational-age (LGA) as being > 97th percentile of BW. Stunting as being < 3rd percentile of the length and wasting as being < 3rd percentile of BMI. Single and multiple log-binomial regression models were fitted to estimate the relative risks of CNMM, comparing them to IG21. Results: 4,072 infants were included. The adjusted relative risks of CNMM associated with selected growth phenotypes were (BNRN/IG21): 1.45 (0.92–2.31)/1.60 (1.27–2.02) for SGA; 0.90 (0.55–1.47)/1.05 (0.55–1.99) for LGA; 1.65 (1.08–2.51)/1.58 (1.28–1.96) for stunting; and 1.48 (1.02–2.17) for wasting. Agreement between the two references was variable. The growth phenotypes had good specificity (>95%) and positive predictive value (70-90%), with poor sensitivity and low negative predictive value. Conclusion: The BNRN phenotypes at birth differed markedly from the IG21 standard and showed poor accuracy in predicting adverse neonatal outcomes.

RESUMO: Objetivo: Descrever os padrões de mortalidade geral e por causas externas e a evolução temporal nos municípios da Bacia Hidrográfica do Rio Paraopeba previamente ao desastre socioambiental de Brumadinho e, adicionalmente, investigar a correlação entre a mortalidade e a privação socioeconômica nesses municípios. Métodos: Foram analisadas estimativas de mortalidade do Estudo Carga Global de Doenças referentes a 26 municípios de Minas Gerais. Calcularam-se taxas de mortalidade geral e por causas externas nos triênios (T) T1 (2000 a 2002), T2 (2009 a 2011) e T3 (2016 a 2018). O coeficiente de correlação de Pearson mediu associação entre as taxas de mortalidade e a privação socioeconômica, segundo Índice Brasileiro de Privação. Resultados: Houve declínio da mortalidade geral na Bacia Hidrográfica do Rio Paraopeba de 717,7/100.000 para 572,6/100.000 hab. e na maioria dos municípios entre T1-T3. A mortalidade por causas externas aumentou de 73,3/100.000 para 82,1/100.000 e foi mais elevada nesses municípios comparando-se com a média do Brasil e de Minas Gerais. As mortes por suicídio e violência interpessoal aumentaram de 29,6/100.000 para 43,2/100.000 na maioria dos 26 municípios. Os acidentes não intencionais reduziram-se no período, e as taxas por acidente de transporte aumentaram. Houve correlação positiva entre a privação socioeconômica e a variação percentual das taxas de mortalidade. Conclusão: Apesar da forte presença da atividade mineradora na região, isso não refletiu na melhoria do quadro sanitário, as causas externas aumentaram no período, associadas às desigualdades, o que deve ser considerado no planejamento para a recuperação das áreas do desastre.

ABSTRACT Objective: To describe the patterns of overall mortality and mortality from external causes and the temporal evolution in the municipalities of the Paraopeba River Basin, before the socio-environmental disaster of the Brumadinho dam and, additionally, to investigate the correlation between mortality and socioeconomic deprivation in these municipalities. Methods: Global Burden of Disease Study mortality estimates for 26 municipalities in the state of Minas Gerais, Brazil, were analyzed. Rates of overall mortality and mortality from external causes were estimated in the triennia (T) T1 (2000 to 2002), T2 (2009 to 2011), and T3 (2016 to 2018). Pearson’s correlation coefficient measured the association between mortality rates and socioeconomic deprivation, according to the Brazilian Deprivation Index (IBP). Results: There was a decrease in overall mortality in the Paraopeba River Basin from 717.7/100 thousand to 572.6/100 thousand inhabitants, and in most municipalities between T1-T3. Mortality from external causes increased from 73.3/100 thousand to 82.1/100 thousand, and it was higher in these municipalities compared with the mean for Brazil and Minas Gerais. Deaths from suicide and interpersonal violence increased from 29.6/100 thousand to 43.2/100 thousand in most of the 26 municipalities. Death rates due to unintentional injuries decreased during the period, and those due to transport injuries, increased. There was a positive correlation between socioeconomic deprivation and the percent change in mortality rates. Conclusion: Despite the strong presence of mining activity in the region, such did not reflect in the improvement of the sanitary situation. Death rates due to external causes increased in the period, associated with inequalities, which must be considered in the planning for the recovery of the disaster areas.

RESUMO Objetivo: Analisar a mortalidade prematura por doenças crônicas não transmissíveis (DCNT) nas capitais brasileiras e Distrito Federal (DF) após redistribuição das causas garbage, e a evolução temporal segundo estratos de privação social nos triênios 2010 a 2012 e 2017 a 2019. Métodos: Foram aplicadas correções ao Sistema de Informação sobre Mortalidade (SIM), sendo empregada metodologia para redistribuição das causas garbage (CG). As taxas de mortalidade prematura por DCNT padronizadas por idade foram estimadas. Foram analisadas as diferenças entre as taxas de mortalidade por DCNT segundo categorias do Índice Brasileiro de Privação (IBP) e entre os triênios. Resultados: No conjunto das capitais, as taxas aumentaram entre 8 e 12% após a redistribuição de CG, e os maiores acréscimos ocorreram em áreas de alta privação: 11,9 e 11,4%, triênios 1 e 2, respectivamente. Houve variabilidade entre as capitais. Observou-se redução das taxas em todos os estratos de privação entre os triênios, sendo maior decréscimo no estrato de baixa privação (-18,2%), e menor no estrato de alta privação (-7,5%). Conclusão: A redistribuição de CG representou aumento das taxas de mortalidade, sendo maior nos estratos de maior privação social. Via de regra, observou-se gradiente positivo de mortalidade com o aumento da privação social. A análise da evolução temporal evidenciou decréscimo da mortalidade por DCNT entre os triênios, sobretudo em áreas de menor privação social.

Methods: Corrections were applied to the Mortality Information System (Sistema de Informação sobre Mortalidade – SIM) data such as the redistribution of garbage codes (GC). Premature mortality rates due to NCDs were calculated and standardized by age. The differences among NCDs mortality rates were analyzed according to the Brazilian Deprivation Index (Índice Brasileiro de Privação – IBP) categories and between the three-year periods. Results: In the capitals as a whole, rates increased between 8 and 12% after GC redistribution and the greatest increases occurred in areas of high deprivation: 11.9 and 11.4%, triennia 1 and 2, respectively. There was variability between the capitals. There was a reduction in rates in all strata of deprivation between the three-year periods, with the greatest decrease in the stratum of low deprivation (-18.2%) and the lowest in the stratum of high deprivation (-7.5%). Conclusion: The redistribution of GC represented an increase in mortality rates, being higher in the strata of greater social deprivation. As a rule, a positive gradient of mortality was observed with increasing social deprivation. The analysis of the temporal evolution showed a decrease in mortality from NCDs between the triennia, especially in areas of lower social deprivation.

ABSTRACT Objective Neck circumference is a simple anthropometric measurement that may be linked to chronic diseases, physical activity, and muscle strength. We sought to verify the association of moderate and vigorous physical activity levels and relative muscle strength with neck circumference in a community in southern Brazil. Methods We cross-sectionally analyzed data from 2,488 participants (51% women), aged 20-79 years old from the Study of Health in Pomerode (SHIP-Brazil) conducted in Pomerode, Santa Catarina, Brazil. Increased neck circumference was defined with cutoff points of >39cm for men and >35cm for women. The independent variables were the level of moderate and vigorous physical activity using the short International Physical Activity Questionnaire, and relative muscle strength using the handgrip test and body mass. Univariate and multiple Poisson regression models were used to determine the association between variables (p≤0.05). Results The prevalence of increased neck circumference was 48.2% (60.4% in men, 39.6% in women) and was associated with low relative muscle strength (PR=1.26, 95%CI: 1.17-1.35) in men, insufficient moderate and vigorous physical activity levels (PR=1.23, 95%CI: 1.14-1.32), and relative muscle strength (PR=1.73, 95%CI: 1.61-1.87) in women. After adjusting for covariates, no significant associations were observed between insufficient moderate and vigorous physical activity levels in men (PR=1.02, 95%CI: 0.95-1.07). Conclusion Increased neck circumference seems to be an important predictor of low moderate and vigorous physical activity and relative strength loss in adults, and more pronounced in women.

Abstract Methane emission from beef and dairy cattle combined contributes around 4.5-5.0% of total anthropogenic global methane. In addition to enteric methane (CH4) produced by the rumen, cattle production also contributes carbon dioxide (CO2) (feed), nitrous oxide (N2O) (feed production, manure) and other CH4 (manure) to the total greenhouse gas (GHG) budget of beef and dairy production systems. The relative contribution in standard dairy systems is typically enteric CH4 58%, feed 29% and manure 10%. Herds with low production efficiency can have an enteric CH4 contribution up to 90%. Digestibility of feed can impact CH4 emission intensity. Low fertility herds also have a greater enteric CH4 contribution. Animals with good feed conversion efficiency have a lower emission intensity of CH4/kg of meat or milk. Feed efficient heifers tend to be lean and have delayed puberty. Fertility is a major driver of profit in both beef and dairy cattle, and it is highly important to apply multi-trait selection when shifting herds towards improved efficiency and reduced CH4. Single nucleotide polymorphisms (SNPs) have been identified for feed efficiency in cattle and are used in genomic selection. SNPs can be utilized in artificial insemination and embryo transfer to increase the proportion of cattle that have the attributes of efficiency, fertility and reduced enteric CH4. Prepubertal heifers genomically selected for favourable traits can have oocytes recovered to produce IVF embryos. Reproductive technology is predicted to be increasingly adopted to reduce generation interval and accelerate the rate of genetic gain for efficiency, fertility and low CH4 in cattle. The relatively high contribution of cattle to anthropogenic global methane has focussed attention on strategies to reduce enteric CH4 without compromising efficiency and fertility. Assisted reproductive technology has an important role in achieving the goal of multiplying and distributing cattle that have good efficiency, fertility and low CH4.

Abstract Many studies about fasciolosis control have been carried out, whether acting on the adult parasite or in Pseudosuccinea columella, compromising the development of the larval stages. The present study aimed to evaluate, under laboratory conditions, the susceptibility of P. columella to Heterorhabditis bacteriophora HP88, during for 24 and 48 hours of exposure. The snails were evaluated for 21 days for accumulated mortality; number of eggs laid; hatchability rate; biochemical changes; and histopathological analysis. We found that exposure induced a reduction in glucose and glycogen levels, characterizing a negative energy balance, due to the depletion of energy reserves as a result of the direct competition established by the nematode/endosymbiont bacteria complex in such substrates. A mortality rate of 48.25% and 65.52% was observed in the group exposed for 24 h and 48 h, respectively, along with significant impairment of reproductive biology in both exposed groups in relation to the respective controls. The results presented here show that P. columella is susceptible to the nematode H. bacteriophora, with the potential to be used as an alternative bioagent in the control of this mollusk, especially in areas considered endemic for fascioliasis, in line with the position expressed by the World Health Organization Health.

Resumo Muitos estudos têm sido realizados sobre o controle da fasciolose, seja atuando no parasito adulto, seja em Pseudosuccinea columella, comprometendo o desenvolvimento das fases larvais. O presente estudo teve como objetivo avaliar, em condições laboratoriais, a suscetibilidade de P. columela à Heterorhabditis bacteriophora HP88, durante 24 e 48 horas de exposição. Os moluscos foram avaliados por 21 dias para mortalidade acumulada; número de ovos postos; taxa de eclodibilidade; alterações bioquímicas; e análise histopatológica. Constatou-se que a exposição induziu a redução nos níveis de glicose e glicogênio, caracterizando um balanço energético negativo, devido ao esgotamento das reservas energéticas, decorrentes da competição direta estabelecida pelo complexo nematoide/bactéria endossimbionte nesses substratos. Foi observada uma taxa de mortalidade de 48,25% e65,52% no grupo exposto por 24 h 48 h, respectivamente, juntamente com comprometimento significativo da biologia reprodutiva em ambos os grupos expostos em relação aos respectivos controles. Os resultados aqui apresentados, mostram que P. columela é suscetível ao nematódeo H. bacteriophora, com potencial para ser utilizada como um bioagente alternativo no controle dess molusco, especialmente em áreas consideradas endêmicas para fasciolose, em linha com a posição expressa pela Organização Mundial da Saúde.

Objectives: to describe the patterns of general mortality and mortality from external causes and the temporal evolution, in the municipalities of the Paraopeba River Basin, before the socio-environmental disaster of Brumadinho and, additionally, to investigate the correlation between mortality and socioeconomic deprivation (PPI) in these municipalities. Methods: mortality databases from the Global Burden of Disease Study in 26 municipalities in Minas Gerais. The researches analyzed the mortality databases from the Global Burden of Disease Study in 26 municipalities in Minas Gerais. General and external causes mortality rates were calculated in the triennia (T) T1 (2000 to 2002), T2 (2009 to 2011), and T3 (2016 to 2018). Pearson's correlation coefficient measured the association between mortality rates and socioeconomic deprivation, according to the Social Deprivation Index (IBP). Results: There was a decline in overall mortality in BHRP from 717.7/100,000 to 572.6/100,000 inhab, and in most municipalities between T1-T3. Mortality from external causes increased from 73.3/100,000 to 82.1/100,000. And they were higher in these municipalities compared to the average for Brazil and Minas Gerais. Deaths from suicide and interpersonal violence increased from 29.6/100,000 to 43.2/100,000 in most of the 26 municipalities. Unintentional accidents decreased during the period, and transportation accident rates increased. The association with socioeconomic deprivation (PPI) was positive with increased mortality rates. Conclusion: Despite the strong presence of mining activity in the region, this did not reflect in the improvement of the sanitary situation, the external causes increased in the period, associated with inequalities, which must be considered in the planning for the recovery of the disaster areas.

Objetivos: descrever os padrões de mortalidade geral e por causas externas e a evolução temporal, nos municípios da Bacia Hidrográfica do Rio Paraopeba (BHRP), previamente ao desastre socioambiental de Brumadinho e, adicionalmente investigar a correlação entre a mortalidade e a privação socioeconômica (IBP) nesses municípios. Métodos: Foram analisadas bases de mortalidade do Estudo Carga Global de Doenças em 26 municípios de Minas Gerais. Foram calculadas taxas de mortalidade geral e por causas externas nos triênios (T) T1 (2000 a 2002), T2 (2009 a 2011) e T3 (2016 a 2018). O coeficiente de correlação de Pearson mediu associação entre as taxas de mortalidade e a privação socioeconômica, segundo Índice de Privação Social (IBP). Resultados: Houve declínio da mortalidade geral na BHRP de 717,7/100.000 para 572,6/100.000 hab, e na maioria dos municípios entre T1-T3. A mortalidade por causas externas aumentou de 73,3/100.000 para 82,1/100.000, e foram mais elevadas nesses municípios comparando-se com a média do Brasil e de Minas Gerais. As mortes por suicídio e violência interpessoal aumentaram de 29,6/100.000 para 43,2/100.00, na maioria dos 26 municípios. Os acidentes não intencionais reduziram no período e as taxas por acidente de transporte aumentaram. A associação com a privação socioeconômica (IBP) foi positiva com aumento das taxas de mortalidade. Conclusão: &nbsp;Apesar da forte presença da atividade mineradora na região, isto não refletiu na melhoria do quadro sanitário, as causas externas aumentaram no período, associadas às desigualdades, o que deve ser considerado no planejamento para a recuperação das áreas do desastre.

Objectives: To analyze premature mortality due to noncommunicable diseases (NCDs) in Brazilian capitals and the Federal District (DF) according to the redistribution of garbage causes and the temporal evolution according to social deprivation strata in the trienniums 2010 to 2012 and 2017 to 2019. Methods: Corrections were applied to the Mortality Information System (SIM) such as: proportional redistribution of ignored and blank data and garbage causes (GC). Municipal premature mortality rates from NCDs were calculated using the local empirical Bayesian estimator and standardized by age. Differences between NCD mortality rates according to the Brazilian Deprivation Index (IBP) categories and between the three-year periods were analyzed. Results: In the capitals as a whole, rates increased between 8 and 12% after the GC redistribution and the greatest increases occurred in areas of high deprivation: 11.9% and 11.4%, triennia 1 and 2. There was variability between the capitals. There was a reduction in rates in all strata of deprivation between the three-year periods, with the greatest decrease in the stratum of low deprivation (-18.2%) and the lowest in the stratum of high deprivation (-7.5%). Conclusion: The redistribution of GC represented an increase in mortality rates, being higher in the strata of greater social deprivation. As a rule, a positive gradient of mortality was observed with increasing social deprivation. The analysis of the temporal evolution showed a decrease in mortality from NCDs between the three years, especially in areas of less social deprivation.

Objetivos: Analisar a mortalidade prematura por doenças crônicas não transmissíveis (DCNT) nas capitais brasileiras e Distrito Federal (DF) segundo redistribuição das causas garbage e a evolução temporal segundo estratos de privação social nos triênios 2010 a 2012 e 2017 a 2019. Métodos: Foram aplicadas correções ao Sistema de Informação sobre Mortalidade (SIM) como: redistribuição proporcional de dados ignorados e em branco, e das causas garbage (CG). As taxas municipais de mortalidade prematura por DCNT foram calculadas pelo estimador bayesiano empírico local e padronizadas por idade. Foram analisadas diferenças entre as taxas de mortalidade por DCNT segundo categorias do Índice Brasileiro de Privação (IBP) e entre os triênios. Resultados: No conjunto das capitais, as taxas aumentaram entre 8 a 12% após a redistribuição de CG e os maiores acréscimos ocorreram em áreas de alta privação: 11,9% e 11,4%, triênios 1 e 2. Houve variabilidade entre as capitais. Observou-se redução das taxas em todos os estratos de privação entre os triênios, sendo maior decréscimo no estrato de baixa privação (-18,2%) e menor no estrato de alta privação (-7,5%). Conclusão: A redistribuição de CG representou aumento das taxas de mortalidade, sendo maior nos estratos de maior privação social. Via de regra, observou-se gradiente positivo de mortalidade com o aumento da privação social. A análise da evolução temporal evidenciou decréscimo da mortalidade por DCNT entre os triênios, sobretudo em áreas de menor privação social.

Abstract Background Lacosamide (LCM) is a third-generation anti-seizure drug approved in Europe and the United States, either as a monotherapy or adjunctive therapy, to treat partial-onset seizures in adults, adolescents, and children. In Brazil, LCM is licensed for treatment only in patients older than 16 years of age. Objective To evaluate a cohort of children presenting with refractory epilepsy who received LCM as an add-on therapy and observe the response and tolerability to the LCM treatment. Methods A retrospective cohort study conducted in a tertiary health care facility, which included 26 children, aged up to 16 years, who presented with refractory epilepsy and received LCM as an add-on treatment. The follow-up visits were scheduled every 3 months until 9 months of treatment with LCM. Results After 3 months of LCM administration, in 73.1% of the children, there was a reduction of > 50% in the frequency of seizures, and this clinical improvement was maintained in most patients (73.9%) for the following 9 months. Mild (such as, somnolence and behavioral changes) or severe (seizure worsening) adverse effects were observed in two and three children respectively. Among responders to LCM, there was a higher prevalence of males, fewer concomitant anti-seizure drugs, and lower percentage of patients using sodium channel blockers. Conclusions Lacosamide should be considered as an early treatment option in pediatric patients with refractory epilepsy, mainly focal seizures.

Resumo Antecedentes Lacosamida (LCM) é um fármaco anticrise de terceira geração aprovado na Europa e nos Estados Unidos, utilizado como monoterapia ou terapia adjuvante para tratar crises epilépticas focais em adultos, adolescentes e crianças. No Brasil, a LCM só é aprovada para tratamento em pacientes com mais de 16 anos de idade. Objetivo Avaliar uma coorte de crianças com epilepsia refratária que receberam LCM como terapia adjuvante e observar a resposta e tolerabilidade ao tratamento. Métodos Um estudo de coorte retrospectivo conduzido em uma unidade terciária de saúde, que incluiu 26 crianças de até 16 anos de idade que apresentavam epilepsia refratária e receberam um tratamento complementar com LCM. As visitas de acompanhamento foram agendadas a cada 3 meses, até 9 meses de tratamento com LCM. Resultados Após 3 meses de administração de LCM, em 73,1% das crianças, a frequência das crises teve uma redução maior do que 50%, e essa melhora clínica foi mantida na maioria dos pacientes (73,9%) pelos 9 meses seguintes. Efeitos adversos leves (como, sonolência e alterações comportamentais) ou graves (agravamento das crises) foram observados em duas e três crianças, respectivamente. Entre as crianças que responderam ao tratamento com LCM, houve uma maior prevalência do sexo masculino, o uso de um menor número de medicações anticrise associadas e o uso de bloqueadores dos canais de sódio. Conclusões A LCM deve ser considerada uma opção de tratamento precoce em pacientes pediátricos com epilepsia refratária, principalmente aqueles que apresentam crises focais.
